E-Counseling works well for those clients who are not able to attend sessions in person, either because of geographic location or disability and do not have a clinician in their geographical area who is experienced with their specific concerns.  Dr. Clark offers E-Counseling for:

Gender exploration

Transgender transition

Sex addiction recovery

General sexuality education/consultation

The process works in a similar way to face-to-face counseling, except it is via an electronic medium. We make an appointment, you fill out forms online, make your payment online, and then I call you at our scheduled time.


I meet with clients on Tuesdays & Wednesdays from 10am until 9pm, Eastern Standard Time (EST). I will initiate the telephone or Zoom call within 5 minutes of your scheduled time and sessions last for 55 minutes. Skype is not a secure medium, so if you have concerns about that, we can have our sessions through WebEx, which is password protected. Let me know if you prefer this prior to our session.

The fee is $200.00 US per session. In some circumstances, after the first session, we may meet for half an hour for a fee of $100.00.  I do not generally accept insurance as most companies do not reimburse for gender dysphoria or sex addiction. I am also not licensed to provide psychotherapy outside of Florida. Florida residents who have a diagnosis other than or in addition to gender dysphoria or sex addiction may be eligible for insurance reimbursement.

We will first discuss what brings you to counseling and what your goals are. I will then either take your history or review the form you completed on line with you. This is the beginning of clarifying what will be addressed in future sessions. Finally, we will discuss how often you will need to meet and what you can expect to accomplish.

This is also the time when we may discuss with you the need for more intensive therapy, which I am not able to provide through E-Counseling. Appropriate referrals will be explored if this is the case.


This question will be better answered after the first session, when we will discuss where you are in your transition and what your goals are. Except in very unusual circumstances, we will need a minimum of three sessions. I promise to always be respectful of your time and finances, while still being ethical and responsible.


My letters of recommendation for hormones and surgery have been accepted by surgeons in the United States, Canada, and Thailand. I have had only one endocrinologist refuse to accept a letter for hormones due to the counseling not being in person, so I suggest that you check with your endocrinologist in advance.

The other day I watched a funny, feel-good movie called Cool Runnings (1993), which was based on the true story about a Jamaican bobsled team trying to make it to the Olympics.  It starred John Candy as the coach, Irv, who was trying to redeem himself after cheating many years previously.  In one of the final scenes, a member of the team asked him why he had cheated, especially since he had already won two gold medals, and Irv’s answer truly embodies a large part of what my book, Addict America: The Lost Connection, is all about.  He says, “It’s quite simple really.  I had to win.  I made winning my whole life, and when you make winning your whole life, you have to keep on winning, no matter what.  A gold medal is a wonderful thing, but if you’re not enough without it, you’ll never be enough with it.”

That is addiction in America in a nutshell.  We have to win, we have to be the best, we cannot be losers or second-rate.  This thinking has propelled us to the moon.  It has made us champs in technology.  It has also made us arrogant and insecure.  It has made us often dissatisfied with what we have and who we are. We are always seeking more – more gadgets, more toys, more money, more power, and more fame.  We want bigger houses, bigger breasts, and bigger penises.  We want faster cars, faster internet, and faster lifestyles.  We want more and more stimulation and that is our addiction.

Irv made the point about internal versus external locus of control.  When you need something external to validate you internally, you will never get enough.  Recovery is about the knowing that you have everything you need right here, right now.  When you are in the moment, you can be content.  When you are present, you can Connect with others.  When you can just be, you can be happy.

The Sex Therapy Training Institute STTI  was created in order to educate and train licensed professionals in the field of sex therapy.  Those individuals licensed in the State of Florida must have this training in order to refer to themselves by the title of “sex therapist.”  Upon completion of the program, those professionals holding a professional license will also be eligible to apply for certification at a national level.

You’ve heard the expression “Nature Abhors a Vacuum,” right? Basically, a vacuum, or empty space, cannot exist in nature. Something has to fill it. If you sucked all the air out of an enclosed space and then opened up a hole, air would be sucked right back in.

The same is true in our lives. When we say “I am not going to….. (take drugs, watch online porn, get angry) we create a vacuum. In the absence of anything else with which to fill that empty space, whatever it was we tried to remove from it will be sucked right back in. That is how Addiction Loves a Vacuum. No matter how hard we try to NOT do our addictive behavior, if we leave a vacuum, the addiction will fill it again.

So we need to frame our desires in a positive way. What do we want to do instead of our addiction? Pretty much everyone is able to do whatever they put their minds to, as long as it is framed concretely. “I will eat dinner with my family,” “I will go to a meeting,” or “I will take an hour-long yoga class.”

It is about Intention – the Intention to live in Recovery, the Intention to do those behaviors that are prosocial and health-promoting.

Start each day with an Intention for how you want your day to be, rather than how you do NOT want it to be.
